Passport Agreements

The Passport Program provides direct funding for adults with developmental disabilities to live more independently and become involved in their communities. An eligible adult with a developmental disability, including youth turning 18 and transitioning to adult services will get at least $5,000 a year of direct funding through the Passport Program. Passport is a reimbursement program that lets an individual manage their own funding. This means that you can choose to pay for eligible services and supports and send invoices for your expenses to be reimbursed up to the approved funding per year.

Community Living Fort Frances and District will provide assistance in managing Passport Funding. Community Living Fort Frances and District does not provide staff to deliver supports and services.

On your behalf if requested Community Living Fort Frances and District will manage some, or all of an individual’s Passport funding directly. Community Living Fort Frances and District will submit invoices for reimbursement and will charge 10% administration fee as part of the funding agreement with Developmental Services Ontario.
